04/17/2003 It is imperative to select the correct option in Item 62. The Aerospace Medical Certification Division (AMCD) has received paper copies of 8500-8 forms with Item 62 checked deferred, but the transmitted version of the same exam is marked issued. This can cause confusion and delay certification for airmen. PLEASE TAKE GREAT CARE WHEN FILLING OUT THE ELECTRONIC FORM.
